For people with diabetes, a 3 – 4 day food and glucose journal can be found here

For people without diabetes, a 3-day food record can be found here

A copy of any recent medical notes, blood results, and a list of any current medications.

Privacy

Your confidentiality is of utmost importance. We will always ask for your consent before sharing or requesting information from your GP or another health provider.

Paper notes kept are destroyed after your session, any electronic notes are recorded on a password-only computer system. The sharing of information between you and us via email does pose a privacy risk.
